A valid driver's license or passport.

GED or high school diploma for students 18+, parental consent form for students ages 16-17.

Copy of your social security card.

Proof of a negative TB test completed within 6 months of the class start date—head to a CVS minute clinic or arc to have this done.

Signed copy of the student enrollment agreement.

Prepared money order or online payment for full program amount (money order or credit card only). Payment installments and discounts are available!

Scrubs to wear for both labs and clinicals. Discount available!

A device with stable internet access and the ability to use Microsoft Teams and Google Classroom

A copy of the textbook and workbook picked up from the clinical site during the date and time provided post-enrollment
