SUBMIT REQUIRED DOCUMENTS
Click the Google Drive Icon and request access to a secure folder. Once you've received an email indicating you've been granted access, drop the following required documents into the folder:
🔴 Valid drivers license or passport
🔴 GED or high school diploma for students 18+, parental consent form for students aged 16-17
(MINIMUM AGE REQUIREMENT IS 16 YEARS OLD PRIOR TO THE FIRST DAY OF CLASS)
🔴 Copy of Social Secuirty card or ITIN
🔴 Proof of negative TB skin test that has been completed within 12 months of the class start date-simply head to a CVS Minute Clinic or ARC to have this done.
🔴 Signed copy of the Student Enrollment Agreement
SOME CLINICAL REQUIRE: PROOF OF HEALTH INSURANCE AND UP-TO-DATE VACCINATIONS. BE PREPARED TO SUBMIT THESE DOCUMENTS AS WELL ONCE THE PROGRAM BEGINS.

SUBMIT TUITION PAYMENT
Several options for tuition payment are available including utilizing payment plans through PayPal and Affirm + simply making a full one-time payment via credit card
Note: It is best to call or email the school to ensure the desired program has seats available prior to submitting forms and payment.
Once all required documents for admission are submitted and tuition is paid, you will be officially enrolled in the program. You will then receive Next Step Information and a Program Guide detailing clinical site information, where to purchase scrubs and other pertinent details about the class
