Required Documents To Be Uploaded Under The Enroll Now Button On The Website​

• A valid driver's license or passport.

• GED or high school diploma for students 18+, parental consent form for students ages 16-17.

• Copy of your social security card.

• Proof of a negative TB test completed within 6 months of the class start date—head to a CVS minute clinic or arc to have this done.

• Signed copy of the student enrollment agreement.

Other Required Items To Have For The Program

• Prepared money order or online payment for full program amount (money order or credit card only). Payment installments and discounts are available!

• Scrubs to wear for both labs and clinicals. Discount available!

• A device with stable internet access and the ability to use Microsoft Teams and Google Classroom

• A copy of the textbook and workbook picked up from the clinical site during the date and time provided post-enrollment

• Ability to read and write English or Spanish
